How to manually file a claim
Make sure you submit your claim with the following information:
Photo of the bar code(s)
Receipt
Purchase date
UPC Code
Where you bought the test(s)

How long until I get my money back?
It depends on your insurer, but expect anywhere between 30 - 50 days from the date you send your claim.
Where can I get at-home COVID tests for free?
You have access to your insurer's preferred network of places to buy a test.
The good news is you can walk into any of the retailers below, show your insurance card, and get up to 8 at-home COVID tests per member per month for free.
The bad news is if you buy tests anywhere else and submit a claim for reimbursement, you'll be reimbursed up to $12 per test, even if you spent more than that per test.
